Physical Therapist - Inpatient Rehab (Travel/Contract)

Location: Allegan, Michigan
Contract Length: 13 weeks

Job Details

• Specialty: Therapy/Rehabilitation (Physical Therapy)
• Work Setting: Mix of Inpatient, Outpatient, and Subacute (Swing) patients
• Patient Focus: Primarily Orthopedics, with possible care for Geriatrics, Pediatrics, and Vestibular patients
• Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM – 4:30 PM)
• Guaranteed Hours: 40 hours per week
• Call, Weekend, and Holiday Requirements: None
• Floating: Required between Inpatient and Outpatient settings

Required Qualifications

• Valid Physical Therapist license in Michigan
• Minimum 1 year of Physical Therapist experience within the last year

Dress Code & Documentation

• Scrub Color: Teal scrubs or business casual
• Charting System: Redoc

Background & Compliance Requirements

• Background check covering last 7 years and all states lived/worked in, required within 90 days of start and annually thereafter
• Education verification or copy of diploma/transcripts required
• Employment application required prior to start
• Michigan LARA fingerprint results required

Certifications

• BLS (Basic Life Support) - AHA only
• ACLS (Advanced Cardiac Life Support) - AHA only (certification requirements may vary by unit)
• PALS, NRP, CHEMO, TNCC, EFM, NIH Stroke Certification, and other certifications may be required depending on unit assignment (refer to unit-specific job descriptions)

Health & Safety Requirements

• Physical exam required prior to start
• Negative TB test or chest X-ray within 1 year (2-step PPD or Q-gold accepted; T-spot not accepted)
• TB questionnaire required prior to start and annually
• Immunizations or titers required for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Varicella
  • Hepatitis B (titer, series, or declination)
• Seasonal flu vaccination or approved medical/religious exemption required prior to start and by December 15 annually
• Fit mask test required within 1 year of assignment start (specific mask types accepted); PAPR training provided if unable to pass fit test
• Negative 12-panel drug screen within 30 days of start (includes amphetamines, cannabinoids, MDMA, methamphetamines, buprenorphine, oxycodone, fentanyl, cocaine, methadone, tramadol, benzodiazepines, morphine/opiate)

Additional Notes

• Candidates must complete facility-specific modules (70-80 modules, approx. 8-10 hours) prior to start; paid time for completion
• Clear, passport-style photo required for badge
• Confidentiality and non-disclosure agreement must be signed before starting
• No annual background check required for advanced practice nurses (not applicable here)
• On-site EKG exam and cardiac rhythm review required only for Monitor Techs (not applicable)
