Location: Hood River, Oregon
Contract Length: 13 weeks

- Scrubs provided for OR, Critical Care, ED, and Maternal Child units (any color scrubs allowed otherwise)

Required Qualifications
- Valid Pharmacist license in Oregon
- Minimum 1 year of Pharmacist experience within the last year
- Experience in an Outpatient setting
Orientation
- Reduced Non-Billable Orientation (0 hours vs. standard 40 hours)
- First-time traveler or 1-year experience preferred
Background and Screening Requirements
- Government-issued photo ID (Driver’s License or State ID)
- Social Security Card (as applicable)
- Background Check within 90 days covering all locations lived/worked in past 7 years; updated annually
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start, updated monthly
- Education Verification required for all candidates
- Drug Testing: 9-panel drug screen within 12 months of start (instant acceptable)
- References: Minimum 2 references covering 1 year of employment in a comparable specialty/setting within last 3 years
- Employment Application: Must include at least last 5 years of employment with month/year format
- Work History Verification required if reference exceptions are requested
Health and Immunization Requirements
- Physical Exam within 1 year of start, signed by MD, NP, PA, or DO
- Fit Mask test (travel only, valid within 1 year or completed onsite)
- Tetanus/Diphtheria/Tdap vaccine within 10 years (declination accepted)
- Negative TB test or chest X-ray within 12 months, updated annually
- MMR (Measles, Mumps, Rubella): 2 vaccines or positive titer, declination accepted
- Hepatitis B: Documentation of vaccination series or signed declination
- Varicella: 2 vaccines or positive titer, declination accepted
- Seasonal Flu Vaccine during flu season (declination accepted)
- COVID vaccination record preferred; declinations not required but unvaccinated must follow infection prevention policies
- Respiratory Safety Clearance (travel only, annual medical clearance for respirator use)
Facility and Compliance Requirements
- Annual completion of regulatory and specialty-specific training modules via LMS (Workramp)
- Photo for badge (passport style with neutral background) required
- Float Policy Acknowledgement: Traveler may be required to float to another facility within reasonable distance
- Epic training modules required and must be completed within a week of orientation
- State-specific background and licensing attestations as applicable
