Scheduling & Parking

Compliance & Documentation Requirements

  • Proof of I9 or Attestation
  • Government-issued photo ID (must be carried during hours of service)
  • E-Verify or Attestation
  • Residency Declaration Form (if working on a compact license)
  • Background Check: 7-year criminal history, nationwide, sex offender, FACIS Level 3, education verification
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start and updated with contract renewals
  • Education Verification through primary source (transcripts/diplomas not accepted as sole proof)
  • Drug Testing: 9-panel drug screen within last 12 months (instant acceptable)
  • Physical Exam: Initial and updated annually, must meet physical demands of position
  • Fit Mask Testing: Within one year of start, updated annually
  • Immunizations and Titers:
    • Tetanus/Diphtheria/Tdap (vaccine or declination form)
    • Negative TB test or chest X-ray within 1 year
    • Rubeola (Measles), Mumps, Rubella (titer or 2 vaccines, booster if needed)
    • Hepatitis B (titer, vaccine series, or declination form)
    • Varicella (titer or 2 vaccines, booster if needed)
    • Flu Vaccine (required annually; religious and medical exemptions accepted with approval)
  • COVID-19 Vaccine: Fully vaccinated required (no exemptions accepted)
  • Additional Health Documents: Eye exam (including vision acuity), color blind exam (mandatory, online exam not accepted)

Competency & History Requirements

  • Medication Test/Pharmacology Competency Exam (as applicable)
  • Core Competency Exams (Parts I, II, III) for subcontractors
  • References: Proof of 2 clinical references or hospital evaluations
  • Employment Application: Must include licensure, certifications, education, and employment history
  • Skills Checklist: Updated annually for applicable units

Additional Notes

  • Clinicians will float within their scope to meet facility needs.
  • No prior travel experience is explicitly required.
  • Must comply with all facility-specific forms and acknowledgments prior to start.
  • The facility does not accept pending background checks or instant statewide searches as placeholders; full clearance is required before starting.

This position is ideal for Registered Nurses with at least 2 years of L&D experience who are fully vaccinated against COVID-19 and meet the certification and compliance requirements listed above.
