Occupational Therapist (Outpatient) - Travel/Contract

Location: Maryville, Missouri
Contract Length: 13 weeks
Shift: Monday-Friday, 8:00 AM – 2:30 PM (40 hours/week)

Job Summary

Provide occupational therapy services in an outpatient setting.

Required Qualifications

• Valid Occupational Therapist license in Missouri
• Minimum 1 year of occupational therapy experience within the last year
• Basic Life Support (BLS) certification (AHA or ARC accepted)

Work Schedule

• Monday through Friday
• 8-hour shifts from 8:00 AM to 2:30 PM

Compliance and Documentation Requirements

• Valid Driver’s License or State ID
• Background check within 30 days of start, including:

  • National Criminal Check
  • State and County Searches (10 years, all aliases)
  • SSN Trace
  • National Sex Offender Registry
  • FACIS Level 3
  • EPLS/GSA/SAM and HHS/OIG checks
• Education verification with official transcript or diploma including graduation date
• Missouri EDL Search within 30 days of start
• Physical exam within the last 12 months (not required annually)
• Drug screen within 30 days of start (not required annually)
• Immunization and health requirements:

  • Flu vaccine required during flu season (or approved medical/religious exemption with mask requirement)
  • Negative TB test or chest X-ray within 1 year (with additional requirements if positive)
  • Proof of vaccination, positive titer, or declination for:

    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
• Fit mask testing completed onsite at orientation
• Two professional references within 3 years
• Employment application, skills checklist, and recent work history verification

Facility-Specific Requirements

• Review and sign confidentiality agreement and code of conduct
• Complete demographic form prior to start
• Complete agency orientation quiz with a minimum score of 84% before starting
• Annual acknowledgment of safety manual and pass/fail testing policies

Additional Notes

• Local candidates within a 50-mile radius are welcome and may be offered a reduced rate
• Onsite fit testing and some training may be required during orientation
