Job Description

• Position: Registered Nurse (RN) - Medical/Surgical
• Employment Type: Travel
• Facility: Acute care hospital setting

Required Certifications & Licenses

• BLS (Basic Life Support) – Must be American Heart Association (AHA) certified
• Nursys Verification – Within 30 days prior to start; disciplinary actions require additional documentation
• Current Nursing License – Primary source verification within 30 days prior to start
• Ascension Color Vision Test – Completed prior to start and updated every 3 years
• Ascension Core Competencies – HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly (minimum 80% score, max 2 attempts)
• NIH Certification – Group A updated yearly, Groups B-F every 2 years
• Ascension EKG Exam – Passing score of at least 80%, max 2 attempts; updated as new editions are released
• Ascension Onboarding Requirements Education Attestation – Completion of pre-hire education modules
• Ascension Nursing Contract Worker Onboarding – Completed onsite prior to start
• Ascension Nova StatStrip Training – Required pre-hire modules for non-allied positions
• MOAB Moderate Risk De-Escalation Training
• Ascension Cerner Test-Out – Cerner system training and test results required
• Ascension Badge Photo – Forward-facing headshot with white or blue background, no alterations

Health & Immunization Requirements

• MMR (Measles, Mumps, Rubella) – Documented proof of two vaccines or positive IGG titer; no declinations accepted
• Fit Test – Required for all travelers; preferred prior to start, can be completed onsite; updated annually
• Hepatitis B – Documentation of vaccination series completion or signed declination
• Influenza Vaccine – Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
• Positive PPD / Negative Chest X-Ray and Annual TB Questionnaire – Positive IGRA or PPD results, chest x-ray within 1 year, and TB questionnaire completed and updated annually
• Health Statement – Statement of good health or physical exam prior to first assignment, updated annually
• 2 Step PPD OR IGRA (Quantiferon/Tspot) – Within 1 year and 30 days of start; TB questionnaire updated annually
• Varicella – Proof of two vaccines, serological immunity, or history verified by healthcare provider; no declinations accepted
• Tdap – Valid for 10 years from administration; no declinations accepted
• Ascension 12 Panel Drug Screen – Within 30 days prior to start; includes testing for 12 specified drugs; must include MRO review if applicable

Professional & Background Requirements

• Driver's License/State ID – Must be current, match address on file, and comply with compact license rules if applicable
• Ascension Profile (Resume) – Required for submission
• Professional References – At least two qualifying references prior to first assignment
• Criminal Background Check – Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender; completed within 30 days of first assignment and updated annually or after breaks in service
• EPLS (Excluded Parties List System) Check – Within 90 days of start; rerun if break in service > 90 days
• Ascension Skills Checklist – Clinical skills checklist completed within one year of start and updated annually
• Education Verification – Verification of relevant education within 90 days of start; official transcripts required if verification fails
• Confirmation of Assignment – Provided upon initial assignment and updated with extensions
• Employment Verification – Third-party verification for past 7 years; updated annually or after breaks in service

Optional / Additional Requirements

• Ascension Unit Specific Checklist – Optional for onboarding but must be uploaded within a week by unit leader
• Hiring Manager Evaluation – Post-assignment evaluation (optional)
• Name Change Documentation – Legal documentation if applicable (optional)
