Job Description

• Assignment at a hospital in Wichita, KS.
• Travel position with a 13-week contract.

Required Credentials and Certifications

Licenses & Verifications

• Current Nursing License with primary source verification within 30 days prior to assignment start.
• Nursys Verification within 30 days prior to start.
• Driver's License/State ID (must be valid and match address requirements).
• Ascension Profile (resume or professional profile).
• Ascension 2 Professional References (at least two professional references).
• Ascension Criminal Background Check (includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender, and more; updated annually or if break in service > 90 days).
• Ascension EPLS (Exclusionary Party List System) check within 90 days of start.
• Ascension Education Verification (verification of education relevant to clinical work within 90 days of start).
• Ascension Employment Verification (third-party employment verification for past 7 years; updated annually or if break in service > 90 days).
• Ascension Confirmation of Assignment (documented start and end dates on agency letterhead).

Certifications

• BLS (Basic Life Support) - Must be American Heart Association (AHA) certified.
• NIH Certification - Updated yearly for Group A, every 2 years for Groups B-F.
• MOAB Moderate Risk De-Escalation Training.
• Ascension Core Competencies - Includes HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly (minimum 80% score, max 2 attempts).
• Ascension EKG Exam - Passing score of at least 80%, max 2 attempts; updated as new editions are released.
• Fit Test - Required annually; preferred prior to start.
• Ascension Skills Checklist - Clinical skills checklist completed within one year of start and updated annually.
• Ascension Cerner Test-Out - Cerner system competency test.

Personal Health History / Immunizations

• MMR - Documented proof of two vaccines or positive IGG titer; no declinations accepted.
• Hepatitis B - Documentation of vaccination series completion or signed declination.
• Influenza Vaccine - Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement.
• Positive PPD / Negative Chest X-Ray and Annual TB Questionnaire - Must provide positive IGRA or PPD results, chest x-ray within a year, and annual TB questionnaire.
• 2 Step PPD OR IGRA (Quantiferon/Tspot) - Within 30 days prior to start.
• Varicella - Proof of two vaccines, serological immunity, or history verified by healthcare provider; no declinations accepted.
• Tdap - Valid for 10 years; no declinations accepted.
• Health Statement - Statement of good health or physical exam prior to first assignment, updated annually.
• Ascension 12 Panel Drug Screen - Includes testing for 12 specified drugs; must be within 30 days prior to start and updated annually.

Onboarding & Training Requirements

• Ascension Color Vision Test - Completed prior to start and updated every 3 years.
• Ascension Onboarding Requirements Education Attestation - Completion of pre-hire education modules.
• Ascension Nursing Contract Worker Onboarding - Onsite onboarding.
• Ascension Nova StatStrip - Pre-hire modules for non-allied positions.
• Ascension Unit Specific Checklist - Optional for onboarding but must be uploaded within a week by unit leader.

• All required documents and verifications must be current and uploaded as specified.
• Some credentials require renewal or re-verification if there is a break in service longer than 90 days.
• Compliance with all immunization and health screening requirements is mandatory.
• Drug screening and background checks are strictly enforced and must meet Ascension Health standards.
