Overview

• Travel/Contract assignment in Dayton, Ohio
• 13-week contract
• Night shift: 4x12 hours, 19:00–7:00
• Unit: Med Surg Renal Unit
• Unit size: 22 beds
• LTAC LPN experience is accepted
• Must be able to work a minimum of every other holiday
• Must be available to work every other weekend/holiday
• Floating is required and traveler will be first to float at the facility’s discretion
• Float scope includes the specialty cluster and, if assigned to the facility, Main Campus (Dayton), South Campus (Centerville), and North Campus (Englewood)

Required Qualifications

• Valid LPN / LVN license in Ohio
• Med Surg (1 year of experience)
• Med Surg experience within the last year
• BLS - Basic Life Support issued by American Heart Association
• IV certification
• Experience in an Acute Care setting
• Experience with Epic
• Current resume/application
• 2 references from within 2 years of start, with 1 from a Manager, Supervisor, or Charge RN
• Skills checklist within 1 year prior to documentation due date and updated annually
• Physical exam within 2 years of start
• Negative TB or chest X-ray
• Drug screen within 12 months prior to documentation due date
• Annual health questionnaire
• Candidate agreement
• E-Verify
• Government-issued photo ID
• Background check
• EPLS/GSA/SAM
• HHS/OIG
• Respiratory protection medical questionnaire
• RespSafety clearance

Additional Notes

• Travelers are not eligible for employee incentives
• The facility may cancel 2 shifts due to low census per 13-week contract
• The agreement may be canceled with 2 weeks’ notice without fee or fine
