Schedule

Days, Monday through Friday, 8:00 AM to 4:30 PM
40 hours per week

Required Qualifications

Valid RN license in Michigan
Associate of Science in Nursing / Associate Degree in Nursing
Clinic experience within the last year
Minimum of one year of experience working with the frail, elderly, or long-term care population
BLS issued by American Heart Association
Driver's License
Experience working with Geriatric patients
Geriatrics experience
Wound Care experience
Home Health experience

Key Responsibilities

Provide direct nursing care to participants
Triaging emergent situations
Monitor acute and chronic medical conditions in coordination with primary care providers
Use care plans independently to provide clinic and home-based direct care
Administer treatments, procedures, and medications as prescribed
Make regular home visits, including wound care, lab draws, nurse assessments, and other nursing tasks

Work Environment

Works independently and collaborates with licensed and non-licensed personnel
Visible presence in the center with frequent interruptions
May work beyond normal hours, weekends, and holidays as needed
