Registered Nurse – Home Health

Location: Antigo, Wisconsin
Employment Type: Travel
Contract Length: 13 weeks

Job Details

• Shift: 8-hour varied shift (typically 7:00 AM - 3:30 PM)
• Guaranteed Hours: 37 hours per week, flexible up to 40 hours
• Weekend Schedule: Every 4th weekend
• Holidays: Rotational coverage
• On Call: 1-2 times during the week and every other weekend; primarily triaging calls with a 15-minute response time
• Patient Load: Approximately 20-25 patients, averaging 4.5 visits per day
• Travel: Around 50 miles per day depending on territory and patient census
• Float: May be required to float to other territories during low census periods

Required Qualifications

• Valid RN license in Wisconsin
• Minimum 2 years of Home Health experience, including experience within the last year
• Two years or more of total travel nursing experience
• BLS certification issued by American Heart Association or American Red Cross
• Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
• Experience with Epic documentation system
• Valid Driver’s License

Clinical Responsibilities

• Conduct OASIS admissions, manage all time points and episode management
• Coordinate care including physician calls and documentation in EMR
• Clinical competencies required in wound care, IV therapy, ports, lab draws, and chronic disease management
• Timely and accurate documentation and coordination of care

Additional Information

• Contractors must live outside a 50-mile radius of the facility
• Must be dependable, accountable, compassionate, resilient, and able to manage change well
• Social and positive attitude preferred
• Orientation week hours may vary; guaranteed hours apply except during holidays when the unit is closed
• Eligible for up to 36 hours of unbillable low census hours; on-call hours count toward this total
• Call back pay applies with a minimum of 2 hours if called back during on-call
• Flu and COVID vaccination required during flu season or mask must be worn at all times per policy; non-compliance may result in corrective action
