Location: Des Moines, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks (approximately 3 months)
Schedule: Every Other Week (EOW)

Required Credentials & Certifications

Licenses & Certifications (Must be current and verified within 30 days of start)

• Registered Nurse License (state verification required)
• BLS (Basic Life Support) – American Heart Association certification required for all direct patient care personnel.
• ACLS (Advanced Cardiovascular Life Support) – Required for RNs in CCU, ED, PACU, MICU, SICU.
• NIHSS (National Institutes of Health Stroke Scale) – Required.
• TNCC (Trauma Nursing Core Course) – Optional.

Professional Requirements

• Signed Trinity Code of Conduct Form
• Completed Trinity I9 form
• Signed Confidentiality of Patient and Proprietary Information and Information System Security Agreement
• Signed Trinity Integrity and Audit Services Form (signed shortly before start)
• Completed Trinity Non Employee Upload Form (pages 1-3 and 28)
• Completed Trinity Conflict of Interest Disclosure Form
• Completed Trinity Criminal Background Check (Federal, Nationwide, County for last 7 years, including SSN verification)
• Completed Trinity Education Verification (highest academic degree or diploma verified)
• Completed Trinity EPLS (Excluded Parties List System) check within 30 days of start
• Completed Trinity GSA/OIG (Office of Inspector General) check within 30 days of start
• Completed Trinity Joint Commission CORE Exam within the last year
• Completed Trinity NSO (National Sex Offender) check within 30 days of start
• Completed Trinity System Access Form (to be submitted one week before start)
• Completed Trinity Skills Checklist (clinical contractors only)
• Completed Trinity Child and Adult Dependent Abuse Training (Iowa only)
• Completed Trinity IA Sing Report (Iowa Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check within 30 days prior to start)
• Completed Trinity OFAC (Office of Foreign Assets Control) check within 30 days of start

Health & Immunization Requirements

• Fit Test (required)
• Trinity 10 Panel Drug Screen (completed within 30 days of start, preferred within 14 days; must include specific substances, NO THC)
• COVID-19 Vaccine (proof of booster by Sept 2025, or declination/exemption with agency letterhead; declination not accepted in CA or IL)
• MMR (proof of 2 vaccinations or IgG titer lab report)
• Hepatitis B (must have started vaccination series prior to start; titers not accepted)
• Influenza Vaccine (proof of vaccine, declination, or exemption; declination not accepted in CA or IL; valid for 10/1/2024 - 4/30/2025)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) (TB screening; positive requires chest x-ray per regional guidelines)
• Varicella (proof of 1 vaccination or IgG titer lab report)
• Physical/Health Statement (exam within last year confirming fitness to work)
• Tdap (current, non-expired vaccine; declinations accepted if no pediatric interaction)
• Trinity Color Vision (required)
