- Confirmation of Assignment (start/end dates on agency letterhead) (required for onboarding)
- Nursing Contract Worker Onboarding (completed onsite)
- Onboarding Education Attestation (completion of pre-hire 6-hour module)
- Ascension Profile (resume or profile)
Licenses
- Current nursing license with primary source verification within 30 days prior to start (required for submission)
- Nursys verification within 30 days prior to start
- Valid Driver's License or State ID (must not be expired and address must match records)
Certifications
- BLS (American Heart Association) (required for onboarding)
- NIH Certification (updated yearly for Group A, every 2 years for Groups B-F)
- MOAB Moderate Risk De-Escalation Training
- Ascension EKG Exam (passing score of 80%, two attempts allowed)
Personal Health and Immunizations
- Ascension Color Vision test (completed prior to start, updated every 3 years)
- MMR: Proof of two vaccines or positive IGG titer (no declinations accepted)
- Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted)
- Tdap: Valid within 10 years (no declinations accepted)
- Hepatitis B: Vaccination series completion or signed declination
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); declinations accepted with mask requirement
- Fit Test: Required for all travelers, preferred before start (annual update)
- 2 Step PPD or IGRA (Quantiferon/Tspot) testing with annual TB questionnaire
- Positive PPD requires negative chest X-ray within 1 year and TB questionnaire
- Health Statement or physical exam prior to first assignment, updated annually
- Ascension 12 Panel Drug Screen within 30 days prior to start, updated annually
Testing and Checklists
- Ascension Core Competencies (HIPAA, Joint Commission, OSHA topics) with minimum 80% score, updated yearly
- Ascension Cerner Test-Out with signed attestation
- Ascension Skills Checklist (clinical only, updated annually)
- Ascension Unit Specific Checklist (optional for onboarding)
- Ascension Nova StatStrip pre-hire modules (required for non-allied positions)
