Job Description

Patient Care Responsibilities

  • Provide safe, competent, quality nursing care based on nursing theory, research, and evidence-based practice.
  • Assess and evaluate patient care needs using critical thinking.
  • Deliver nursing care integrating assessment and intervention skills.
  • Collaborate with interdisciplinary healthcare teams to develop, implement, and evaluate care plans.
  • Prioritize patient care including teaching, rounding, coaching, discharge planning, and delegation.
  • Educate patients and caregivers on care plans, transitions, health promotion, and disease prevention.
  • Communicate effectively and professionally with patients, families, and healthcare team members.
  • Manage assignments within the Modified Primary Nursing Care Delivery Model.

Patient Experience

  • Role model Relationship Centered Care principles including admission welcome, bedside shift report, purposeful hourly rounding, medication review, discharge wrap-up, and collaboration with physicians.
  • Respect patient values, preferences, cultural diversity, and expressed needs.
  • Establish and maintain therapeutic relationships with patients and families.
  • Use evidence-based practices to understand patient perceptions of care.
  • Initiate service recovery and escalate concerns as needed.
  • Demonstrate caring, respect, compassion, empathy, and active listening.
  • Support staff in evidence-based patient care practices.

Quality and Safety

  • Practice safe, efficient, patient-centered care and identify barriers to improvement.
  • Demonstrate clinical skills and knowledge essential to quality and safety.
  • Implement process improvement strategies and participate in peer and institutional reviews.
  • Collaborate on quality improvement projects, evidence-based practice activities, and nursing research.
  • Contribute to compliance with quality and safety standards.
  • Apply high reliability principles to avoid high-risk behaviors.
  • Escalate safety hazards following institutional protocols.
  • Document nursing practice accurately to support quality and performance improvement.

Operations

  • Perform charge nurse duties as assigned.
  • Coordinate with nursing leadership to ensure appropriate use and inventory of supplies.
  • Escalate material or staffing shortages as needed.
  • Practice cost-effective patient care regarding equipment, staff, and supplies.
  • Document thoroughly and in compliance with regulatory and legal standards.
  • Facilitate patient throughput during shifts.

Professional Development

  • Support clinical colleagues, patients, families, and healthcare team members.
  • Advance clinical competence from novice to expert.
  • Maintain a professional image and communicate with respect, compassion, and empathy.
  • Incorporate ethical principles in decision-making.

Qualifications & Skills

  • Current New York State RN License required.
  • Basic Life Support (BLS) certification.
  • Minimum 2 years RN experience.
  • Minimum 2 years experience in Post Open Heart and Cardiac Care Unit.
  • Travel experience preferred but not required for Per Diem positions.
  • EPIC electronic health record experience highly preferred.
  • Bachelor of Science in Nursing (BSN) preferred; Associate degree considered with relevant experience or active BSN matriculation.
  • Ability to work 12-hour shifts with weekends required.
  • Unit census averages 26 beds.
  • Staffing ratio approximately 1 nurse to 2-3 patients.
