Home Health RN Case Manager
Job Overview
- Location: Baltimore, Maryland
- Work setting: Onsite
- Employment type: Contract to hire
- Contract length: 13 weeks
- Schedule: Monday–Friday, 8:00 AM–5:00 PM
- Hours: 40 per week
- OT: Possible with prior approval
- Coverage area: Baltimore City and Baltimore County, with possible coverage in Anne Arundel County and Howard County if needed
Key Responsibilities
- Perform comprehensive assessments for high-risk members
- Collaborate with primary care providers to implement individualized care plans
- Reassess care needs and coordinate appropriate levels of care
- Arrange non-medical support to help with treatment plan compliance
- Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals
- Engage specialty resources as needed
- Maintain detailed clinical, functional, and fiscal records
- Support health promotion and illness prevention
- Help prevent adverse patient outcomes and intervene quickly when needed
- Protect patient privacy and confidentiality
Required Qualifications
- Valid RN license in Maryland
- Case Management (1 year of experience)
- Case Management experience within the last year
- BLS
- Case Management
- Associate of Science in Nursing / Associate Degree in Nursing
- Experience in a Home Health setting
- Driver's License
- Wound Care experience
- Experience with OASIS
Systems / Tools
- Point of Care / HomeCare HomeBase
- OASIS
