Description

Summary The Clinical Liaison maintains a regular and ongoing presence with SNF and Home Health Agencies to build and maintain strong, trusting, and professional working relationships to improve total post-acute cost of care and improve patient quality and satisfaction. Minimum Qualifications • Education: Graduate of an accredited school of nursing (Associates of Nursing or Bachelor of Nursing) • Licenses/Certifications: Current, unrestricted licensure to practice as a Registered Nurse • Experience / Knowledge / Skills: o 1-2 years' experience in a post-acute in a Healthplan or SNF with knowledge of utilization management and CMS criteria. This is a remote position and there is no Home Health or Home visits to the patient’s home. This position will work with Hospitals Acute Case Management, SNF and HH to help determine medical necessity appropriateness for the right care in the right setting and at the right time through the use of evidence based criteria performing Utilization reviews and working with the PAC team and other clinicians and support staff to help achieve desired performance outcomes.

Nicole was very helping and always proficient with reaching out and even going above and beyond when the facility itself was slacking. When the facility canceled all contracts the day before start date she found me another position within the same week. I would recommend her 10x over.
