RN Care Manager

Location

Schedule

• Days, 8 hours per day, 40 hours per week

Contract Length

• 13 weeks

Role Summary

• Provides ongoing support through assessment, planning, implementation, and evaluation of patient needs.
• Focuses on case management, utilization review, and discharge planning for acutely ill patients with complex medical conditions.

Required Qualifications

• Valid RN license in North Carolina
• Associate of Science in Nursing / Associate Degree in Nursing
• 2 years of Case Management experience
• Case Management experience within the last year
• Experience in an Acute Care setting
• Acute Hospital Case Management Experience
• Care Manager experience

Preferred Qualifications

• BSN or MSN from an accredited school of Nursing

Key Responsibilities

• Review work lists to prioritize patients and identify new admissions
• Conduct and document assessments and plans of care in Epic
• Participate in daily care management touchpoints
• Attend CAPP meetings and Complex Care Meetings
• Coordinate discharge planning, transition planning, and post-discharge referrals
• Communicate barriers to discharge and support progression of care
• Coordinate family meetings and provide education on community resources
• Monitor observation patients throughout the day
• Support medication resource management for non-resourced patients as needed
