Description
RN Care Manager
Location
- Chapel Hill, North Carolina
Schedule
- Days, 8 hours per day, 40 hours per week
Contract Length
- 13 weeks
Role Summary
- Provides ongoing support through assessment, planning, implementation, and evaluation of patient needs.
- Focuses on case management, utilization review, and discharge planning for acutely ill patients with complex medical conditions.
Required Qualifications
- Valid RN license in North Carolina
- Associate of Science in Nursing / Associate Degree in Nursing
- 2 years of Case Management experience
- Case Management experience within the last year
- Experience in an Acute Care setting
- Acute Hospital Case Management Experience
- Care Manager experience
Preferred Qualifications
- BSN or MSN from an accredited school of Nursing
Key Responsibilities
- Review work lists to prioritize patients and identify new admissions
- Conduct and document assessments and plans of care in Epic
- Participate in daily care management touchpoints
- Attend CAPP meetings and Complex Care Meetings
- Coordinate discharge planning, transition planning, and post-discharge referrals
- Communicate barriers to discharge and support progression of care
- Coordinate family meetings and provide education on community resources
- Monitor observation patients throughout the day
- Support medication resource management for non-resourced patients as needed
