Description
The Manager of Transitional Care is responsible for the direction and leadership of operational, financial, programmatic and personnel activities for Transitional Care services at Tampa General Hospital (TGH). This includes establishing, meeting and continuously monitoring the department goals and objectives while maintaining alignment with TGH strategic goals and objectives. Directs the day-to-day Transitional Care and Think Ambulatory First operations, budgeting, financial management and human resource management. Partners directly with the Manager of Nurse Navigation to oversee the continuum of transitional care and navigation service delivery. Works closely with a variety of stakeholders, coordinating the activities of transitional services across the enterprise. Provides direction and leadership in the development, implementation, oversight and evaluation of care coordination programs in collaboration with TGH Care Continuum, Case Management, Ambulatory Care Coordination, TGH Homecare, TGH Post-Acute Care (PAC) Preferred Provider Network, TGMG Geriatrics, Population Health and other community health facilities/agencies. Responsible for advancing strategies to optimize the transition of patients across the continuum of care, working with leaders and teams to develop partnerships, coordinate clinical processes and establish/evaluate performance metrics. Responsible for performing job duties in accordance with mission, vision and values of Tampa General Hospital.
Essential Functions:
- Manages and directs all activities within area of responsibility.
- Incorporates TGH’s vision, mission and values in goals and programs within Transitional Care services.
- Supports AKTiVe Leadership (Authenticity, Kindness, Transparency and Vulnerability); develops trust and fosters an inclusive team member environment.
- Supports a culture of safety by promoting Zero Harm and High-Reliability.
- Prepares and delivers reports to operational leadership outlining progress toward meeting annual goals and objectives, to include performance related to finance, clinical activity, quality and safety.
- Maintain records related to operations and services that are complete, accurate, available and in compliance with all legal, regulatory and policy requirements.
- Supports the development of Transitional Care initiatives to improve quality, patient satisfaction and patient/family-centered care.
- Communicates with physicians, academic department leaders and senior leaders to maintain coordination with other TGH programs.
- Manages the complex interdepartmental and interdisciplinary relationships to assure collaboration and effective/efficient operations within Transitional Care.
- Bachelor's Degree - Nursing
- Florida Registered Nurse License
- Ambulatory Care Nursing Certification (AMB-BC) through American Nurses Credentialing Center (ANCC) or equivalent certification required within a year.
- Minimum five (5) years of clinical nursing experience. One (1) to two (2) years of experience supervising or managing Transitional Care services.
