Description
Pride Health is actively hiring a Registered Nurse (RN) to join our client's Home Health Team.
Location: St. Louis, MO, 63141
Pay: $52.00/hr W-2
Contract: 14 Weeks | Potential Extension
Schedule: Monday–Friday | 9:00 AM–5:00 PM
Hours: 40 hrs/week | 8-hour shifts
Job Summary:
- We are seeking an experienced Registered Nurse (RN) to provide skilled nursing care to patients in their homes.
- The RN will assess patient needs, develop and manage Plans of Care, coordinate with interdisciplinary teams, educate patients and families, and ensure quality and continuity of care.
Key Responsibilities:
- Perform comprehensive assessments of home health/hospice patients and develop, implement, and evaluate Plans of Care.
- Provide skilled nursing care, including wound care, IV/venipuncture, catheter/drain care, phlebotomy, inhalation therapy, and pain management.
- Maintain accurate and timely clinical documentation and patient records.
- Educate patients and families on medications, diet, safety, disease management, and self-care.
- Coordinate care with physicians, patients/families, caregivers, and interdisciplinary team members.
- Supervise and provide clinical direction to HHA/STNA/LVN staff as appropriate.
- Ensure compliance with payer, infection control, and regulatory requirements.
- Participate in interdisciplinary team meetings and provide on-call services as assigned.
- Travel locally to patient homes and work independently while maintaining effective communication with the care team.
Required Qualifications:
- Current Registered Nurse (RN) license in the state of practice.
- 2+ years of Home Health experience required.
- 1–2 years of clinical nursing experience required.
- Homecare Homebase (HCHB) experience – Required.
- OASIS experience – Required.
- Current CPR certification – Required.
- Valid driver's license, reliable transportation, and auto insurance.
- Strong clinical assessment, communication, organization, and documentation skills.
- Ability to work independently and travel locally.
