We are seeking a compassionate, dependable, and resident-focused Registered Nurse (RN) to join our long-term care team in Plymouth, WI. The RN will provide high-quality nursing care to residents, promote their health and well-being, and work collaboratively with nursing staff, providers, residents, and families to ensure the highest standards of care.

The ideal candidate demonstrates strong clinical judgment, excellent communication skills, and a commitment to maintaining residents' dignity, independence, and quality of life.

Essential Duties and Responsibilities

• Provide direct nursing care and oversee the delivery of care to residents in accordance with individualized care plans.
• Assess residents' health status, identify changes in condition, and initiate appropriate interventions.
• Administer medications and treatments safely and accurately in accordance with physician orders and facility policies.
• Communicate with physicians, nurse practitioners, residents, families, and interdisciplinary team members regarding changes in resident condition and care needs.
• Supervise, support, and provide direction to licensed practical nurses (LPNs) and certified nursing assistants (CNAs), as applicable.
• Participate in care planning, resident assessments, and ongoing evaluation of care outcomes.
• Complete accurate and timely documentation in the electronic health record or medical record.
• Respond appropriately to emergencies, falls, infections, and other changes in resident condition.
• Follow infection prevention, medication administration, safety, and quality assurance procedures.
• Support resident rights, confidentiality, dignity, and person-centered care practices.
• Assist with admissions, transfers, discharges, and coordination of services as needed.
• Maintain compliance with applicable state and federal regulations and facility policies.

Qualifications

• Current, unrestricted Registered Nurse (RN) license in the State of Wisconsin.
• Current CPR certification or willingness to obtain as required by the facility.
• Long-term care, skilled nursing, rehabilitation, or geriatric nursing experience preferred.
• Strong assessment, clinical decision-making, and organizational skills.
• Excellent verbal and written communication skills.
• Ability to work effectively as part of a multidisciplinary team.
• Compassionate approach to resident care and a commitment to professional nursing standards.
