Pay Range: $90,000 - $120,000
$5000 Sign On Bonus
Schedule
Full Time | Monday-Friday | 8:00 AM-5:00 PM

Are you an experienced Hospice RN, Hospice Director of Nursing, Hospice Clinical Manager, or Clinical Leader ready to make a meaningful impact?
Phoenix Home Care & Hospice is seeking a Hospice Director of Nursing (DON), RN to lead our clinical team in Kanas City, MO and the surrounding Kansas City area.
This is an opportunity to lead talented clinicians while ensuring patients and families receive exceptional hospice, end-of-life, palliative, and comfort care.
Requirements
- Active Registered Nurse (RN) license
- Minimum 2 years of hospice or skilled home health leadership experience
- Strong knowledge of hospice nursing, end-of-life care, Medicare hospice requirements, and clinical compliance
- Strong leadership, communication, and team development skills
- Experience as a Hospice DON, Hospice Clinical Director, Hospice Clinical Manager, Hospice RN Case Manager, or Hospice RN preferred
- Passion for compassionate, high-quality patient care
Responsibilities
- Lead hospice clinical operations and patient care services
- Supervise, mentor, and develop Hospice RNs, LPNs, CNAs, and clinical staff
- Ensure quality patient-centered hospice and end-of-life care
- Oversee clinical documentation, chart audits, quality, and compliance
- Ensure compliance with Medicare hospice regulations, state requirements, and CHAP standards
- Support hospice admissions, assessments, recertifications, and plans of care
- Collaborate with physicians, patients, families, and the hospice interdisciplinary team (IDG)
- Support clinicians through education, onboarding, and clinical guidance
- Complete patient visits as needed
- Build strong relationships with referral partners and community providers
