Job Profile Summary
Job Summary:

The Case Manager, RN - Palliative is responsible for the assessment, planning, implementation, and evaluation of palliative patient referrals within the MWHC Emergency Departments. The incumbent in this position will collaborate with providers to ensure well-coordinated, multi-disciplinary programs of care. This position will also allow for increased coordination of care, streamlining end of life processes, reduction of re-admission rates, and improved attention to goals of care.

Essential Functions & Responsibilities:

  • Identifies and screens palliative care patients for care coordination.
  • Works with Emergency Department (ED) staff to coordinate planning and care needed for palliative patients.
  • Communicates status updates regarding treatment plans with clinical partners as needed.
  • Coordinates the care of patients with clinical partners; helps patients and family members advance towards realistic and desirable outcomes.
  • Manages patient/family/physician expectations. Provides information and education on community resources to patients and their families.
  • Assists with palliative related education for patients, nursing staff, and physicians.
  • Develops, coordinates, and communicates discharge plans with the patient, family members, and care team. Promotes communication and coordination of care amongst care providers.
  • Organizes and expedites treatment plans. Documents assessment and overall discharge plan in the medical record.
  • Performs other duties as assigned.

Qualifications:

  • Valid RN Licensure in the Commonwealth of Virginia
  • Associate’s or Bachelor’s degree in Nursing, required.
  • Certified Hospice and Palliative Nurse, required within 2 years of hire.  
  • Minimum of two years of experience in end-of-life care or hospice/palliative care, preferred.
