As a Home Health Physical Therapist, you will:
Evaluate patients’ mobility, strength, balance and functional abilities in their homes
Develop individualized treatment plans based on each patient’s needs and personal goals
Provide hands-on therapy to improve movement, safety and independence
Adjust treatment plans as patients progress and their needs change
Educate patients and caregivers on exercises, equipment, fall prevention and safe mobility
Collaborate with physicians, nurses, therapists and other members of the care team
Guide support staff in helping patients achieve their functional goals
Complete accurate and timely clinical documentation
Serve as a trusted therapy resource for patients, families, colleagues and the community
Graduate of an American Physical Therapy Association-approved physical therapy program
Current, unrestricted Physical Therapist license in the state of practice
Dependable vehicle, valid driver’s license and current automobile insurance
Ability and willingness to travel throughout the assigned coverage area
Ability to work independently while remaining connected to a collaborative care team
Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
