Description
American Traveler is hiring a Case Manager RN for an inpatient care management role requiring BSN and prior care management experience.
Details
- Inpatient hospital setting within the Care Management department
- Oversee a daily caseload of inpatient patients with varied care needs
- Attend interdisciplinary rounds and complete initial patient assessments
- Collaborate with patients, families, caregivers, and the healthcare team to develop plans of care
- Coordinate post-discharge needs such as home health and inpatient rehab with post-acute referral sources
- Document and place orders for home health, DME, and nutritional needs within Epic EMR
- Day shift schedule, Monday through Friday, 8:00 AM to 4:40 PM
- No weekend or on-call requirements
Requirements
- Active RN license (NH or Compact) required
- BSN required
- Certified Case Manager (CCM) certification required
- Minimum 3 years of inpatient clinical experience
- At least 1 year of Care Management experience
- Recent Level 1 or comparable Level 2 Trauma experience within the last 2 years
- Epic EMR experience preferred
- BLS certification required

- Candidates must reside outside a 75-mile radius of the facility to be considered
