Registered Nurse – Case Management- Case Manager - Travel - (CM RN)
StartDate: 5/21/2024 Available Shifts: 8 D Pay Rate: $2463.68 - $2715.08

Kaiser Permanente Medical Center, San Jose, CA is an acute care hospital with 279 licensed bed capacity. Clinical services provided at the medical center include but are not limited to emergency, non-surgical cardiac care, cath lab, intensive care, telemetry, medical surgical, labor and delivery and OB, full service perioperative services and ambulatory surgery unit. This department is staffed with RN Patient Care Coordinators who each have expertise in both utilization management and case management. These services are provided for all bedded units and the emergency department and include medical necessity assessment, discharge planning and care coordination. There is strong collaboration with the Physicians and clinical providers to ensure that our excellent clinical care is provided for all patient at the appropriate level of care and coordinated with post hospital services through our integrated health system.

Required Qualifications
Case manager /Critical care nurse, medical surgical nursing is part of the case management with nursing perspective for this facility. CLN needs discharge planning and utilization review.

Preferred Qualifications
Graduate of an accredited school of nursing; BSN preferred. Current California RN license required. Certifications: BLS and ACLS required. Two (2) years of relevant clinical nursing experience relevant to given dept and two (2) years clinical leadership or supervisory experience; Knowledge of Nurse Practice Act, The Joint Commission, and other local, state, federal regulations. Demonstrated strong interpersonal communication skills.
