Job Description:Supplemental Health Care is hiring experienced Home Health LPNs (1+ year in a Medicare-certified agency) for contracts in Hawthorne, New York, and nearby areas. Qualifications:<ul><li>Active, unencumbered LPN/LVN license in the state of assignment</li><li>Current BLS certification</li><li>1+ year of recent experience (within 3 years) in home health, hospice, or skilled nursing</li><li>Skilled in wound care, injections, med administration, and vitals under RN supervision</li><li>Familiar with OASIS-E functional domains</li><li>Clear background check, abuse registry, and driving record</li><li>Health clearance with TB and required immunizations</li><li>Reliable transportation and valid auto insurance</li><li>Willing to travel within the assigned area and adapt to patient needs</li></ul>Home Health Nurse Contract Details:<ul><li>$1,540 - $1,710 per week*</li><li>13-week contract</li><li>SHC Home Health Nurses are paid by the hour, not by the visit</li><li>Visits typically last 30 to 60 minutes depending on the care plan.</li><li>LPNs are expected to conduct 6–8 visits per day based on task complexity and travel requirements.</li><li>Travel time is incorporated into productivity planning and may vary based on geographic spread.</li></ul>Key Responsibilities:<ul><li>Provide skilled nursing services under the direction and supervision of an RN.</li><li>Monitor patient status and immediately report changes to the supervising RN or physician.</li><li>Accurately document all care provided in compliance with agency policies and state nurse practice acts.</li></ul>We understand home health care is unique and requires the right support for success.
