Description
At St. Luke’s, our dedicated team of Hospice RN Coordinators/Liaisons strive to build a positive, supportive, and inclusive culture that delivers exceptional patient experiences.
St. Luke's Hospice is a Medicare-certified program that provides care within the Treasure Valley. The care provided by our team allows people to pass with dignity in their own homes, in familiar surroundings, with those they love by their side. As an RN Coordinator/Liaison, you will foster the relationship between patients and the facilities to identify the next level of care. You will perform clinical assessments and screening, establish patients' eligibility for care, communicate with families, and provide education on the benefits of hospice care. The person needs to be able to work autonomously and as a team, have interpersonal and communication skills, and have a true passion and desire to care for end-of-life patients to ensure their needs are met.

What you can expect from this role
- Work/life balance | Full Time Days (Mon-Fri 8 hour shifts) with rotating weekends
- Works autonomously and with multidisciplinary team
- Collaborate to design individualized care for adult patients
Qualifications
- Education: Nursing Degree
- Experience: 2 years of RN experience; Hospice or Acute Care experience helpful
- Licenses/Certifications: Current RN License in the State of Practice. Current Basic Life Support (BLS) Certification Provider Certified through American Heart Association or American Red Cross
- Valid Driver's License and proof of Auto Insurance
