Schedule: Monday - Friday, 8:30 AM - 5:00 PM

Requirements:

• Active California RN license
• Current BLS certification through the American Heart Association
• Minimum 2 years recent RN Case Management experience
• Experience in Long-Term Acute Care (LTAC) or Short-Term Acute Care Hospital (STACH) settings required
• Demonstrated experience with discharge planning across acute care environments
• Strong interdisciplinary team collaboration skills
• MCG guidelines knowledge and application
