Speech & Language Pathologist - Home Health - (SLP - HH)
StartDate: ASAP Available Shifts: 8 D Pay Rate: $1587.00 - $1828.00

Required: Able to use laptop for documentation, as well as a cell phone. Proper infection control techniques. SLP license, Driver’s License, BLS Home health SLP - This position will evaluate patients and use professional judgment to design the individualized plan of care which best meets the patient's needs. The Speech Pathologist is responsive to physician referrals, determines the skilled and medically necessary needs of the patient and provides discipline specific interventions to improve the safety and functional independence of its clients through the rehab process. 5-7 points per day (number of patients could vary based on visit type such as Start of Care vs. Routine Visit) EMR is Epic We typically admit between 380-390 new patients per month Average Monthly Patient Census around 420
