Opportunity:

RN - Home Health

Job Details:

• Work Monday through Friday from 8:00 AM to 6:00 PM with one rotating day off each week
• No weekend rotation, on-call duties, or holiday requirements
• See 2-3 patients per day on average
• Monitor queue and consult with patients referred from hospitals or skilled nursing facilities for hospice care
• Provide assessments and recommendations for proper hospice settings for patients transitioning to hospice in hospital settings
• Travel between 1-2 Aurora hospitals per day with average driving time of 45 minutes one way
• Could travel to Aurora Sinai, Aurora St. Luke's, Aurora Grafton, Aurora Hartford, Aurora St. Luke's South Shore, and Aurora Summit Hospitals
• Minimum 2 years of experience required, with hospice liaison experience preferred
• Required certifications: BLS, valid driver's license, auto insurance, and personal vehicle
• EPIC experience required for charting and secure chat functions
• Must be comfortable with symptom management for end-of-life care including pain and shortness of breath
• Must be comfortable with end-of-life medications and educating providers on management
• Must be comfortable speaking and collaborating with physicians
• Must be able to provide education on hospice benefits and philosophy
• Must understand different levels of hospice care and services
• Work with adult patients with heart failure, cancer, and other end-of-life disease processes
• Dress code: Business casual or professional scrubs with white coat preferable but not required
• Free parking provided
• 40 hours of unit orientation with staff member provided
