Inpatient Continuing Care Responsibilities: responsible for providing discharge planning and care management services for acute care patients at Doctors Community Hospital Inpatient Continuing Care at Doctors community hospital Responsibilities': responsible for providing discharge planning and care management services for acute care patients at Virginia Hospital Center Float department (if needed): Holy Cross Hospital-Silver Spring/Holy Cross hospital-Germantown PEDS exp or Med/Surg or Tele ICU/CCU or PCU
