Location: 1101 Van Ness Ave San Francisco, CA 94109

Duration: 26 weeks

Shift: Day 4x10-Hour (08:00 - 18:30), every Thursday-Sunday, no on-call needed.

Requirements:
- CA RN Licensure
- 2 years of Case Management experience
- Utilization Management experience in working with attending physicians supporting level of care protocols, 

- Managed care experience as a Case Manager.

Responsible for Care Coordination and Care Transitions Planning throughout the acute care patient experience. This position works in collaboration with the Physician, Utilization Manager, Medical Social Worker and bedside RN to assure the timely progression and transition of patients to the appropriate level of care to prevent unnecessary admissions or readmissions. The Care Management process encompasses communication and facilitates care across the continuum through effective resource coordination
