Position Title: RN – Case Management
Location: New York, NY 10037
Start Date: April 19, 2026
Shift: 9:00 AM – 5:30 PM
Schedule: 5 Days per Week
Hours: 40 Hours per Week
Job Type: Contract
Position Summary:
We are seeking an experienced and detail-oriented Registered Nurse (RN)—Case Management—to support care coordination and utilization management activities. The RN Case Manager will be responsible for assessing patient needs, coordinating care services, and ensuring appropriate resource utilization while maintaining compliance with healthcare regulations and quality standards.
The ideal candidate will possess strong clinical knowledge, communication skills, and experience working with interdisciplinary teams to ensure effective patient care planning and discharge coordination.
Key Responsibilities:
- Perform case management and utilization review to ensure appropriate patient care and resource allocation
- Assess patient needs and coordinate services across multidisciplinary healthcare teams
- Collaborate with physicians, nurses, and social workers to develop and implement patient care plans
- Facilitate discharge planning and coordinate community resources when needed
- Monitor patient progress and ensure continuity of care
- Maintain accurate documentation in accordance with regulatory and organizational standards
- Participate in interdisciplinary meetings and care planning discussions
- Ensure compliance with clinical guidelines and utilization management protocols
- Communicate effectively with patients, families, and healthcare teams regarding care plans and services
Required Qualifications:
- Minimum two (2) years of experience in Case Management, Resource Management, or Utilization Management
- Active Registered Nurse (RN) License – New York State
- Primary Source Verification of RN License
- BLS Certification – American Heart Association (AHA)
- ACLS Certification – AHA (if applicable)
Schedule Details:
- Shift: 9:00 AM – 5:30 PM
- 40 hours per week
- 5 days per week
