POSITION SUMMARY - Case Manager
POSITION DUTIES - Care Coordination, Discharge Planning, Utilization Review
MINIMUM REQUIRED QUALIFICATIONS -2+ Years case management experience
This position does not require an RN, it could be a case manager with another clinical background such as social worker.
(2 years of rehabilitation experience preferred.)
PRODUCTIVITY EXPECTATIONS - Daily caseload 14 -18
LENGTH OF ASSIGNMENT - 13 Weeks
SHIFT / HOURS PER WEEK - Days, M-F 8a-5p (No Weekends)
SYSTEMS - Cerner
START DATE - 10/12/2026
