Assesses the patient and prepares an individual Master Treatment Plan and monthly reviews and updates thereafter. Performs mini-mental status exam, psychosocial assessments, facilitates both individual and group therapy, documenting all interactions in a time frame compliant with company, state, and federal guidelines. Is responsible for contacting any patient who is absent for 3 or more days to determine reason for the absence and when they plan to return to the program.<ul><li>Must able to perform crisis evaluations and discharge planning</li><li>Performs a mini-mental status exam and psychosocial assessment on all new patients. </li><li>Performs other duties as assigned. </li><li>Provides group and individual therapy and prepares timely documentation in compliance with company, state, and federal guidelines. </li><li> Performs related duties as required</li></ul>Required:<ul><li>Education: Master's degree in Social Work or related field.</li><li>Experience: <ul><li>2 years of related experience. </li><li>Work experience may not substitute for minimum education requirement.</li></ul></li><li>Licensure/Certification: <ul><li>Currently licensed, certified, or registered to practice profession as required by law, regulation in state of practice or policy</li><li>CPR</li></ul></li></ul>
