RN Case Manager
Full Time Weekend Option 364 White Oak Street Nursing Days w/Call Performs and/or coordinates the assessment, planning, implementation, education and evaluation of the patient care provided in the home setting. Responsible for performance within the scope of the Nurse Practice Act of North Carolina. Performance is consistent with hospital and home health policies and State and Federal Guidelines. Is competent to deliver care to clients in Home Health as evidenced by the completion of the RN skills competency checklist. Graduate from an approved school of nursing. BSN in Nursing preferred and/or Diploma in Nursing Required. Current RN licensure in the State of North Carolina and valid NC Driver's License. Current BCLS certification required; specialty certification preferred. Knowledge and application of professional nursing theory, practices and procedures. Knowledge of patient assessments which requires recognition of changes in patient conditions and understanding varied data which could complicate or jeopardize successful recovery. Ability to assess nursing needs of acute and chronically ill patients and their families. Ability to recognize changes, symptoms, or other problems related to a patient's condition and determine appropriate intervention. Ability to counsel patients and their families following the patient's plan of care. Ability to explain surgical or other treatment procedures, medications, personal care and other health related needs. Ability to establish and maintain working relationships with patients, families, physicians and other health care personnel, visitors, and the public. Knowledge of state and federal guidelines regarding Home Health Care. Knowledge of community resources available to enhance the care of patients. Ability to have reliable transportation with current driver's license and proof of liability insurance coverage. Ability to work independently in isolated areas or unkept homes with possible rodent infestation and little or no utilities, non-judgementally. At least one year home health case management experience required.
