<h3>Assignment Overview</h3><ul><li>Shift: Days, 5x8hrs</li><li>Hours: 40 hrs/wk</li><li>Start Date: Jun 22, 2026</li><li>Length: 13 weeks</li><li>Openings: 4</li></ul><h3>Description</h3><p>American Traveler is seeking a Home Health Physical Therapist with at least 1 year of skilled home health experience for a 13-week travel assignment in the Medford, OR area.</p><h3>Details</h3><ul><li>Home health setting serving Medford, OR and surrounding areas</li><li>5x8-hour day shifts (8:00 AM – 4:30 PM); 4x10 and 3x12 schedules are not available</li><li>Caseload of up to 6 patients per day with a standard productivity expectation of 30 points</li><li>1:1 patient ratio</li><li>Charting is done in Epic and OASIS</li><li>No weekend or on-call requirements</li><li>Travelers are expected to float between all facilities within the state</li><li>Occasional patient visits may require driving up to 50+ miles, though the system is designed to minimize drive time and windshield time</li><li>Local candidates are accepted; candidates within a 50–75 mile radius are considered local and are eligible at the same rate</li></ul><h3>Requirements</h3><ul><li>Minimum 1 year of skilled home health PT experience required</li><li>Current BLS certification required</li><li>Epic EMR experience required</li><li>OASIS experience required</li><li>Active PT license required (pending license accepted); license required if applicable</li><li>Must have reliable personal transportation</li><li>Primary COVID-19 vaccination series plus one booster required</li><li>Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting</li><li>SSN and DOB required for consideration</li></ul><h3>Additional Information</h3><ul><li>First-time travelers will not be considered for this position</li><li>Former permanent staff must wait at least 3 months before returning as a traveler and must not reside within 50 miles of the submitting facility</li><li>Any RTO requests should be disclosed upfront, as post-offer RTO requests may be denied</li><li>Any color scrubs are acceptable</li><li>Parking is free at facility locations</li><li>Mileage reimbursement is provided at the IRS standard rate</li></ul>
