Job Description
The RN Case Manager is a key member of a multidisciplinary care management team and is responsible for coordinating care and services for assigned patient populations. This role focuses on comprehensive patient assessment, care coordination, discharge planning, and collaboration with healthcare teams and external payers to ensure continuity of care.
Requirements
Graduate of an accredited school of nursing
Active RN license (Oklahoma or multistate Compact/eNLC)
BLS certification (American Heart Association)
Minimum 3 years of acute care clinical nursing experience
Location: Oklahoma City, OK, 73104
