Position Type: Travel / Temporary Assignment
Specialty: Registered Nurse – Case Management
Shift: 8-hour day shifts (8:00 AM – 4:30 PM)
Hours: 40 hours per week
Pay: $80 per hour
13 weeks contract with possible extension.
Position Overview
We are seeking an experienced Registered Nurse Case Manager for a travel assignment supporting hospital-based utilization review and discharge planning services. The Case Manager will collaborate with interdisciplinary teams to ensure safe, timely, and cost-effective care transitions while maintaining compliance with regulatory and organizational standards.
Key Responsibilities
• Coordinate and manage patient care across the continuum, including admission, inpatient stay, and discharge
• Perform utilization review and discharge planning in collaboration with physicians, nursing staff, and ancillary departments
• Assess patient needs, develop individualized discharge plans, and arrange post-acute services
• Communicate with patients and families regarding care plans, resources, and transitions
• Ensure compliance with regulatory requirements, payer guidelines, and organizational policies
• Document case management activities accurately in the electronic medical record
• Participate in interdisciplinary rounds and care coordination meetings
• Advocate for patient-centered, cost-effective care delivery
Required Qualifications
• Active Registered Nurse (RN) license (state license required)
• Current BLS certification
• Previous hospital-based case management experience preferred
• Strong knowledge of utilization management and discharge planning processes
• Excellent communication, critical-thinking, and organizational skills
Additional Information
• Travel assignment with weekly timekeeping
