Home Health / Hospice RN

Location

• Beaver Dam, Wisconsin

Contract Length

• 9 weeks

Schedule

• Monday through Friday, 8:00 AM to 4:30 PM
• Weekend work only during on-call shifts
• On-call: 2 times per week, Monday through Thursday

Required Qualifications

• Active RN role
• BLS certification
• Travel experience
• Critical thinking
• Chronic disease management
• Patient and family education
• NPWT
• Ostomy care
• Central line care
• IV administration
• Complex wound care
• Hospice experience with pain and symptom management, end-of-life care, empathy, and compassion

Patient Population / Caseload

• Primarily geriatric home care and hospice patients, with occasional pediatrics
• Typical conditions include heart disease, chronic respiratory disease, CHF, COPD, pneumonia, dementia/Alzheimer’s, depression, stroke, cancer, kidney disease, diabetes, liver disease, wounds, joint replacement, surgery, medication management, and catheter management
• About 5–6 patients per day, or 25–30 points per week

Responsibilities

• Routine and OASIS visits
• Weekly IDT meetings and office case conferences
• Monthly all-staff meetings
• Both home health and hospice visits

Work Environment / Equipment

• No vents
• CPAP and BiPAP may be used occasionally, with patients self-managing

Orientation

• 1 day of computer-based training
• 2–5 days of field training/shadowing, depending on experience

Dress Code

• Solid-color scrubs without patterns or logos unless company-issued
• Closed-toe, non-slip shoes with socks or hosiery

Additional Notes

• No floating
• Must clock in and out at the facility
• If the clinician has ever worked for this employer, there must be an end date at least 6 months before submission
