Job Overview

Location: Monmouth, Illinois
Setting: OSF Holy Family Medical Center
Shift: Days
Schedule: Monday–Friday, 8:00 AM–4:30 PM, with some flexibility on start and end times
Contract Length: 13 weeks

Required Qualifications

BLS certification
Active Illinois physical therapy license, or confirmation that the application process has been initiated

Submission Requirements

Last four digits of SSN
Middle initial
Requested time off

Additional Notes

Local placements are accepted within a 50-mile radius
Candidates within 50 miles of the facility are subject to a local rate adjustment
Drug screen must be negative for all panels
12 hours of non-billable orientation are included
Call-off policy allows 12 hours every 13 weeks without penalty or shift repayment
