Local placements accepted within a 50-mile radius

Required Qualifications

Active Illinois license, or confirmation that the application process has been initiated
ACLS
BLS
NIHSS

Submission Requirements

Last four digits of clinician’s SSN
Clinician’s middle initial
Requested time off

Additional Notes

Drug screen must be negative for all panels
12 hours of non-billable orientation
Guaranteed hours do not apply during orientation
Local rate adjustment applies to clinicians with permanent addresses within 50 miles
