Description

Physical Therapist - Home Health

Location

Oakland, California
Territory: Sacramento, CA, with expansion as needed

Schedule

40 hours per week
Monday-Friday
One weekend a month rotation
Day shift

Contract Length

13 weeks

Required Qualifications

Valid Physical Therapist license in California
1 year of Physical Therapist experience
Physical Therapist experience within the last year
BLS
Experience in a Home Health setting

Responsibilities

Prioritize start of care visits
Supervise Physical Therapy Assistants
Submit start of care visit reports the next day
Complete clinical notes and other documentation within 48 hours of the visit
Maintain timely communication with supervisor, intake, and office
Be responsive during normal business hours

Training

3 full days of training: one in-house and two in the field
