Community Hospice

Overview

Contract role in Grand Junction, Colorado
Weekday schedule: Mon-Fri 8am-5pm
40 hours per week
Contract length: 12 weeks

Role Summary

Primary Registered Nurse with an assigned caseload of patients
Leads the interdisciplinary team caring for those patients
Provides hospice care wherever patients call home, including private homes, nursing homes, group homes, and assisted living facilities
Occasional back-up for weekends and on-call

Required Qualifications

Valid RN license in Colorado
Associate of Science in Nursing / Associate Degree in Nursing
BLS
Hospice experience within the last year
Hospice (2 years of experience)
