Position Overview
We are seeking a dedicated RN Care Manager to join our care management team. This position is responsible for ensuring efficient discharge planning and facilitating care coordination for patients.
Work Schedule
- Monday through Friday
- Occasional weekend hours available, not required
Key Responsibilities
- Perform comprehensive discharge planning for patients.
- Coordinate patient care effectively across various settings.
- Collaborate with multidisciplinary teams to meet patient needs.
- Support the Care Management department and assist with coverage as needed.
Qualifications
- Experience: Minimum of 5 years in nursing with a focus on discharge planning and care coordination.
- Skills: Strong organizational and problem-solving abilities. Must possess a calm demeanor and a flexible, can-do attitude.
Patient Care
The RN Care Manager will work with a patient ratio of 1:20-24, handling various patient cases requiring effective management and care coordination.
Dress Code
- Scrubs or professional casual attire acceptable.
Overall Fit
We are looking for a flexible, easy-going problem solver who can work collaboratively within a team to enhance patient care.
