Overview

We are seeking a skilled Discharge Planner to assist the medical team with discharge planning, evaluating durable medical equipment (DME) needs, and providing patient follow-up recommendations. The ideal candidate will have experience in inpatient acute hospital settings and possess a valid WA State License.

Key Responsibilities

• Collaborate with the medical team to develop and implement effective discharge plans for patients.
• Assess patient needs for durable medical equipment and ensure appropriate arrangements are made prior to discharge.
• Provide recommendations for patient follow-up care to ensure continuity of care post-discharge.
• Work closely with patients and their families to communicate discharge plans and address any concerns.
• Document discharge plans and assessments accurately in the electronic medical record.
• Participate in multidisciplinary team meetings to discuss patient progress and discharge readiness.

Qualifications

• Valid WA State License is required.
• Experience in an inpatient acute hospital setting is mandatory.
• Strong communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment.
• Proficient in electronic medical record documentation.
• Knowledge of community resources and support services for patients.
