Overview
We are seeking a skilled Discharge Planner to assist the medical team with discharge planning, evaluating durable medical equipment (DME) needs, and providing patient follow-up recommendations. The ideal candidate will have experience in inpatient acute hospital settings and possess a valid WA State License.
Key Responsibilities
- Collaborate with the medical team to develop and implement effective discharge plans for patients.
- Assess patient needs for durable medical equipment and ensure appropriate arrangements are made prior to discharge.
- Provide recommendations for patient follow-up care to ensure continuity of care post-discharge.
- Work closely with patients and their families to communicate discharge plans and address any concerns.
- Document discharge plans and assessments accurately in the electronic medical record.
- Participate in multidisciplinary team meetings to discuss patient progress and discharge readiness.
Qualifications
- Valid WA State License is required.
- Experience in an inpatient acute hospital setting is mandatory.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team-oriented environment.
- Proficient in electronic medical record documentation.
- Knowledge of community resources and support services for patients.
