Job Description

We are looking for two experienced RN Care Managers for a 13-week assignment. These positions will require a commitment of 40 hours per week, with variable shifts available either from 8:30 am to 4:30 pm or four 10-hour shifts from 7:30 am to 5:00 pm. Although there is no call requirement, weekend and holiday rotations are mandatory. The RN Care Manager will play a crucial role in coordinating concurrent utilization management activities across the organization and serve as the primary link between the patient, healthcare team, organization, and payers during the patient's hospital stay.

Duties and Responsibilities

• Conduct thorough assessments of patient needs, identify resources necessary for transitioning patients to the next care setting, and secure required resources.
• Collaborate with, educate, and lead an interdisciplinary team of professionals during daily care rounds, care conferences, and care planning.
• Demonstrate excellent communication skills in both written and verbal formats, as well as through electronic systems such as email and software applications utilized within the organization.
• Engage with community organizations to arrange for durable medical equipment, secure health insurance authorizations, and coordinate transportation.
• Participate actively in the management of inpatient hospital population length of stay.
• Possess knowledge of hospital reimbursement methods and quality measures relevant to the care management department.

Education, Licensure, Certifications, and Qualifications

• Associate or Bachelor’s degree in Nursing with 3-5 years of clinical experience in medical/surgical nursing, home care, hospital case management, or prior utilization review experience.
• Experience with utilization review and discharge planning is required.
• Certification from a nationally recognized case management or utilization review organization is preferred.
• Must possess active Basic Life Support (BLS) certification. Certification in Case Management (CCM) or Accredited Case Manager (ACM) is preferred.

Shift Information

Shifts are variable, either from 8:30 am to 4:30 pm or four 10-hour shifts from 7:30 am to 5:00 pm. Weekend and holiday rotation is required.

• BLS/BCLS certification
• Certification in Case Management is preferred
