Job Description
We are seeking an experienced Licensed Clinical Social Worker (LCSW) to provide essential support and coordination in a challenging healthcare environment. The ideal candidate will have a strong background in acute rehabilitation, discharge planning, interdisciplinary collaboration, and care coordination.
Key Responsibilities
- Conduct assessments and develop care plans for patients in acute rehabilitation settings.
- Coordinate with interdisciplinary teams to ensure effective discharge planning and implementation.
- Provide case management services to help patients navigate their healthcare needs.
- Facilitate communication among healthcare staff, patients, and their families.
- Implement strategies that promote patient-centered care and optimize recovery.
Qualifications
- Minimum of 2 years of experience in acute hospital settings required.
- At least 1 completed travel assignment in a related role.
- Valid LMSW or LCSW license must be in hand.
- BLS certification required.
- Experience with care coordination and discharge planning highly preferred.
Requirements
All employees must receive the flu vaccine as a condition of employment.
Schedule
This position offers a daytime schedule, working Monday through Friday, from 8:00 AM to 4:30 PM.
