Description

American Traveler is hiring a Pediatric Case Management RN with in-patient case management experience and at least 2 years of RN experience for a day-shift role with no weekends, holidays, or on-call.

Details

• Pediatric Case Management position at a main hospital facility
• Will float to all pediatric units including NICU, PICU/PCICU, Onc, and 9N/S and 10N/S units (no float to psych)
• Day shift schedule, 8:30 AM to 5:00 PM, Monday through Friday
• 40 hours per week with no weekends or holiday coverage required
• No on-call responsibilities

Requirements

• Active RN license (MD or Compact) required
• Minimum 2 years of RN experience required
• BLS certification required
• Current in-patient case management experience required
• Experience with post-acute facility referral platforms such as NaviHealth or AllScripts required
• Pediatric experience not required, but case management and discharge planning experience is required
• Bedside experience preferred, ideally 5+ years
• ICU experience preferred but not required
• Epic EMR experience preferred
• Case management certification preferred
• Experience at a comparable facility size (500+ bed count) required

Additional Information

• Facilitate discharge planning and coordinate post-acute care referrals for pediatric patients
• Collaborate with multidisciplinary care teams across various pediatric units
