Registered Nurse / Case Manager

Shift Details

• 5x8s; Monday–Friday from 8:30 AM to 5:00 PM
• 40 hours per week

Weekend Requirement

• Agency staff may be scheduled up to every other weekend based on staffing needs
• One holiday per year is required, in line with staff expectations

Floating Requirements

This role requires floating within scope to meet facility needs, primarily focusing on the Emergency Department and medical inpatient units. Assignments may vary based on staffing needs and may include coverage for multiple inpatient units.

Time Off and Scheduling

• One pre-approved time-off request is permitted per contract
• The system will automatically approve time off requests of 14 days or less
• Every other winter and summer holiday is required, and no time off requests will be approved during these periods
• No time off on hard-to-fill days, such as Easter, Mother's Day, Father's Day, etc.

On-Call Requirement

No on-call duties are required for this position.

Patient Ratios

• Typical assignment involves one 20-bed unit in partnership with a Social Worker
• During crisis staffing, the RN Case Manager may support approximately 1.5–2 units in collaboration with Case Management and Social Work

Experience Requirements

• Minimum of 2 years of hospital inpatient RN Case Management/discharge planning experience is required
• Experience in Med-Surg and/or Critical Care discharge planning is necessary
• Therapy-only backgrounds will not be considered
• Candidates must be able to perform independently after limited orientation

First-Time Applicants

First-time applicants are welcomed provided they possess the necessary inpatient Case Management experience.

Certification Requirements

• Basic Life Support (BLS) certification is required

Licensure Requirements

An active RN license in Maryland is required prior to submission.

Unit Overview

This position focuses on providing Case Management coverage for emergency department and medical inpatient units, supporting adult and geriatric patients. The Case Manager will work collaboratively with Social Work to coordinate discharge planning and facilitate patient transitions.

Additional Experience Requirements

• Acute hospital Case Management experience is essential
• Strong skills in inpatient discharge planning are required
• Candidates should be comfortable managing medically complex Med-Surg and Critical Care patients
• Experience in coordinating discharge needs, resources, and transitions of care is necessary

Charting System

The charting system utilized is Cerner.
