Description

American Traveler is hiring an RN Patient Care Coordinator for a Pediatric Complex Care Program, requiring strong current pediatric experience and an active RN license (MD or Compact).

Details

  • Hybrid role within the Pediatric Complex Care Program, combining up to 3 in-person days with 2 remote days per week
  • Patient population includes infants, children, and adolescents
  • Coordinates care and clinically manages patients and caregivers
  • Schedule includes five 8-hour day shifts per week with no weekends, nights, call, or holidays
  • Responsibilities include patient education, reviewing after-visit summaries, managing EPIC In Basket messages, triaging calls, and reviewing charts
  • Does not include medication reconciliation or refill management

Requirements

  • Active RN license (MD or Compact) required
  • BLS certification required
  • Strong current pediatric nursing experience required
  • EPIC and case management experience preferred
  • Experience in a comparable large facility setting (500+ bed count) required for consideration

Additional Information

  • Coordinates with DME providers, nursing agencies, and other vendors to address barriers to patient care
  • Writes family communications and outreach letters as part of care coordination
  • Reviews patient charts and manages ongoing communication with families
  • Local candidates accepted at the same pay rate
