Description

RN Case Manager needed with minimum 5 years of recent RN Case Management experience in an LTAC or STACH setting, plus experience with MCG, discharge planning, and strong IDT collaboration.
Must have AHA BLS certification and Indiana or multi-state RN license attached to submission.
Schedule is Monday–Friday, 8a–5p.
Responsibilities include care coordination, utilization review, discharge planning, resource management, patient advocacy, and supporting continuity of care.
